We respect your right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible advice and care.
This information is only available to those involved in your care and you should never be asked for personal medical information by anyone not involved in your care. You have a right to know what information we hold about you. If you would like to see your records, please ask to speak to the Practice Manager.
We will only give access to your medical records to anyone outside the practice if this is necessary for your care (in case of referral etc) or if you have given your explicit consent in writing. Solicitors acting on your behalf will usually require sight of your complete medical records and this should be clearly stated on the consent form.
Should you personally require a full copy of your medical record, you can request this by completing the form below and sending it to the practice. We can scan your entire medical record and transfer it to CD. This service will incur a charge of up to £50.00 depending on the overall volume of your records. It can take up to 4 weeks to process such a request.
