CHANGES TO PARTNERSHIP

Dr Ken Uncle will retire at the end of March 2012. The pace & scale of changes in the NHS and the massive bureaucracy and control imposed on general practice have caused him to throw in the towel.

The practice is very pleased to announce that Dr R. Randhawa, a committed, experienced and well liked local GP will join Dr Lawrence, Dr Garcia and Dr Hanna from April 2012

In order to use this service, you will need to register at reception. You will be given a unique ID and password to log into our appointments website

Please do not wait until you are out of medication before requesting a repeat prescription!

We do not accept requests for repeat prescriptions by telephone. This is to avoid errors with your medication request and to avoid prolonged engagement of the telephone lines.

If you do not wish to order your repeat prescription online with the form above, please use the right hand side of your previous prescription (white background) and tick all the items you require. Alternatively, you can write down your request but please make sure that you include the drug name and form (eg. Lasix capsules), strength (eg. 40mg), dosage (how many a day) and the amount required as well as your full name and date of birth. For your convenience we can provide a form for you to fill in (available at reception). Please keep a copy of this form for future requests

You can hand your request in personally, fax it to us, send it by post or fill in the online form above.   If you include a stamped, self addressed envelope we will post it back to you.

If you are registered for booking appointments online (available to all patients) you can order on the same website as booking appointments and prescription details are pre-entered for you.

Please allow 2 full working days (48 hours) for your prescription to be ready plus additional time for post.
